Registration for 2008-09

Registration for 2008-09 began in early February. 

Families currently enrolled in the program will receive their packets

at that time. 

Families with first graders or those new to the program need

to call Cheryl Antos at (708) 636-4373

to make arrangements to register.

Please bring a copy of your child's Baptismal certificate with you when you come to register, even if your child was baptized at Saint Linus Church.  In addition, for your child's emergency form you will need to know your child's doctor's name, complete address, telephone number, your insurance number, any allergies or special needs your child may have, the date of your child's last tetanus shot and an emergency number where we can reach a friend, relative or neighbor should you be unavailable when needed.

Completed packets may be dropped off at the rectory

anytime before April 15th.
